When you choose an intraocular lens, you are not choosing between "good" and "bad." You are choosing between different sets of benefits and compromises. Every IOL design — monofocal, EDOF, multifocal, or anything in between — makes optical decisions that affect how you see.

A lens that covers more distances must split light in more ways. A lens that keeps all light focused at one distance cannot cover other distances. A lens that smooths the transition between distances may soften the clarity at each individual distance.

These are not flaws in engineering. They are fundamental properties of optics. No lens, natural or artificial, can defy physics.

The fundamental trade-off

The human eye's natural lens is remarkable. It changes shape to focus on objects at different distances — a process called accommodation. As we age, the lens stiffens and loses this flexibility, leading to presbyopia.

An artificial intraocular lens cannot change shape. It is a fixed optical element. Therefore, every IOL must make a choice about where to place its sharpest focus, and how to handle the distances on either side.

What monofocal lenses compromise

A monofocal lens places all its optical power at one distance — usually far. This gives you the sharpest possible vision at that distance, with excellent contrast and minimal side effects.

The compromise is that other distances require glasses. For many people, this is an entirely acceptable trade-off. They value the clarity and simplicity of monofocal vision and do not mind wearing reading glasses.

What multifocal lenses compromise

Multifocal and trifocal lenses split incoming light into multiple focal points. This allows you to see at different distances without glasses.

The compromises are real: some light is lost to each focus, reducing overall contrast. The multiple focal points can create halos and rings around lights at night. The brain must learn to interpret the new visual information, which takes time and does not happen equally for everyone.

What EDOF lenses compromise

EDOF lenses use a different approach — they stretch the range of clear vision rather than creating separate focal points. This generally means fewer halos than multifocal lenses, but the near vision may not be as strong.

The compromise is that while EDOF gives you a continuous range, the clarity at each specific distance may be slightly softer than a monofocal set precisely for that distance.

The right question to ask

Instead of asking "Which IOL is best?" the better question is: "Which compromises am I most willing to live with?"

If you strongly want to minimize glasses and are willing to adapt to some night visual effects, a full-range lens may suit you. If you value the sharpest possible image and do not mind reading glasses, a monofocal may be your best choice. If you want something in between, EDOF or enhanced monofocal may be worth discussing.
